LAMP Health and Medical Information Form (complete every year)
Canada Criminal Record form (complete every 3 years)
(This form is for Canadian residents only. United States residents must contact their state patrol to get a copy of their criminal record check. This information needs to be received by the LAMP office every 3 years.)
All of these forms can be mailed by an individual or as a team. Many teams are looking for additional members. Therefore, if you are not already on a team, please feel free to apply to the LAMP VBS program. You will be assigned to an existing team.
